WAIVER

1.I hereby Grant Global Center for Social Dynamic Research and its legal representatives, the irrevocable right and permission to record, use, publish, stream, and distribute photographs, images, videos, and audio recordings of me captured during the registered event in any media now known or hereafter developed.

This consent includes, but is not limited to:

• Permission to interview, film, photograph, tape, or otherwise make a video reproduction of me and/or record my voice;

• Permission to use my name; and

• Permission to use quotes from the interview(s) (or excerpts of such quotes), the film, photograph(s), tape(s), or reproduction(s) of me, and/or recording of my voice, in part or in whole, in its publications, on its website, for broadcasting, and in all other forms of media.

2.By checking this box I release and forever discharge Global Center for Social Dynamic Research its affiliates, officers, directors, employees, volunteers, agents, sponsors, and any other associated parties (collectively, "Released Parties") from any and all liabilities, claims, demands, or causes of action that I may have now or in the future against the Released Parties, arising from or related to any loss, damage, injury, or death that I may suffer as a result of my participation in the event (including during or after the event), whether caused by the negligence of the Released Parties or otherwise.

3.By registering for and participating in the registered event, I affirm that I am fully aware of the health risks associated with physical activities and the potential for unexpected medical issues or injuries that may occur as a result of my participation. I attest that I am medically fit to engage in the activities related to the event and have not been advised otherwise by a qualified medical professional.

I hereby release, waive, discharge, and covenant not to sue the event organizers, sponsors, representatives, and agents, as well as other participants, hosting facilities, trainers, and if applicable, owners and lessors of premises used to conduct the event (collectively the "Releasees"), from any and all liability to me, my personal representatives, assigned heirs, and next of kin for any and all claims, demands, losses, or damages on account of any injury, including but not limited to the death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise.

4.By checking this box, I, as the parent or legal guardian of my minor guest(s)/guest(s) hereby consent to the terms of the Full Personal Injury Release Form, and Full Medical Release Form on behalf of my minor guest(s)/guest(s).

I acknowledge that I have read and understood the terms and that I am signing on behalf of my minor guest/guest(s)voluntarily.

5.By checking this box, I, as the parent or legal guardian of my minor guest(s)/guest(s) hereby consent to the terms of the Image and Video Release Form. I acknowledge that I have read and understood the terms and that I am signing on behalf of my minor guest(s) voluntarily.
