PERSONAL INJURY RELEASE DOCUMENT

I, hereby acknowledge that I am voluntarily participating in this athletic event, fully aware of the risks involved. These risks may include, but are not limited to, personal injury, property damage, or other losses of any kind.

In consideration of being permitted to participate in the event, I release and forever discharge MySamay, its affiliates, officers, directors, employees, volunteers, agents, sponsors, and any other associated parties (collectively, "Released Parties") from any and all liabilities, claims, demands, or causes of action that I may have now or in the future against the Released Parties, arising from or related to any loss, damage, injury, or death that I may suffer as a result of my participation in the event (including during or after the event), whether caused by the negligence of the Released Parties or otherwise.]

I agree to the following terms:

1. I am solely responsible for my health and safety during the event and will take all necessary precautions.

2. I will adhere to all the rules, regulations, and instructions provided by the event organizers and their representatives.

3. This release covers all claims, including those arising from negligence, strict liability, or any other legal theories.

I declare that I have read and fully understand the contents of this Release Form. I am aware that this is a release of liability and a contract between myself and the above-mentioned parties. By accepting the terms and conditions on the registration form, I acknowledge that I am signing this release form of my own free will.
